Annual Mandatory Education

We understand that not everyone has the availability to come into the office for the mandatory in-service each year. This page will help you complete the majority of your requirements on-line.

For the mandatory education and the driver safety training, there is a verification form that you will need to sign and send back in to the volunteer office once you have reviewed the materials. Please find the links below to both of those requirements. The DMV Record Check form is also available below.

The flu shot is mandatory for all Augusta Health Hospice volunteers. If you are unable to make the annual in-service, please call the employee health office at (540) 332-4725 to schedule an appointment to receive your vaccine. The vaccine is free of charge to all volunteers. If you opt not to receive the vaccine, you will need to sign the flu declination form below and wear a mask during flu season if working with hospice patients. If you receive your vaccine elsewhere, please sign the declination form stating that information. Please send your completed declination form into the volunteer office.

If you are a patient care volunteer that assists with "hands on care" or "mealtime assistance," and are due for your update, please contact the volunteer office (540) 332-5574 to see when the next training will take place.
